Your scholarship mission is renewed on an annual basis.  Please provide us with the following documents to enable us to prepare a report on your behalf for your behalf to renew
your scholarship mission:

1. Confirmation of your appointment to the next level of training.

2. In-training evaluation reports for the current academic year.

3. Advise us of the dates and/or the results of your residency-training program’s board/certification

examinations.

4. Dentists: Please provide this office with an official copy of your transcript of grades at the end of each academic term.
